Couple Things- DNR rarely actually exists. Look above in the Lounge, there's a very detailed thread on this, with very specific criteria. If a provider requests that, you can abide, or not. There can be legitimate reasons for that request, or, simply horrible service etc that they don't want publicized. In this case, probably the latter. Her reviews were very bad a few years ago. I'd like to be optimistic, but I'm very doubtful.
